The downloadable Order Form full size version of what you see here above is a pdf format document. It comes intentionally without (U.P.) Unit Price. Hence if you wish to download the Order Form, you should have already the locked Excel format latest version of the Order Form with the most recent prices. The pdf document is for fax orders to be sent to our sales office (305) 731-2134.

The Excel format version can be emailed back as an attachment with your quantity ordered.

Please identify yourself if you are:

a) a gift shop that wishes to retail our candle holders

b) a hotel/restaurant.Your inquiries are most welcome. Should we have a nearby gift shop that is our authorized retailer, we will refer you to such store to service you better.

On the Excel format Order Sheet, we have made your work much easier and made sure you cannot make any mistake. You can only enter your quantity next to the picture and reference of the product of your interest. Once you have completed your order you can click on "Invoice" and fill out the necessary information so we may bill you appropriately and ship you to your desired address.

The Order Form is secured and will only allow typing in the appropriate column your desired quantities in each line item.

Each time you will enter a new quantity or modify it, your total will be automatically recalculated for you.

Then with a click of a button save your Order Form and attach it to your email addressed back to us. We will do the rest for you making sure your order is delivered to your door steps the soonest possible.
